This Agreement is between Octopus Royal Appliances Repair & Services (“Octopus Royal”, “we”, “us”) and the customer named in this form (“Customer”, “you”) for the Appliance Protection Plan selected on this page.
By submitting this form, you are enrolling in one of the following:
The exact plan, pricing, and any additional surcharges are shown and agreed on the form at the time of enrollment.
By signing and submitting this form, you authorize Octopus Royal and its payment processor (Stripe) to automatically charge your selected Plan fee, any approved high-end/custom surcharges, and applicable service call deductibles to your payment method on file, on a recurring monthly basis, in accordance with these Terms & Conditions.
By checking the agreement box, providing your payment details, and submitting this form, you confirm that you have read, understood, and agree to these Terms & Conditions, including the minimum 12-month term, monthly recurring billing, and any additional high-end or custom charges recorded on this Agreement.
